Step 1
Referring provider will download and fill out the genetic counseling referral form.
Step 2
Provider sends the referral form, along with any relevant medical documents not already provided with the order, to Referral@GeneDx.com or via fax to 201-605-6582.
Step 3
The GeneDx patient counseling team will contact the patient to schedule an appointment. If we cannot reach the patient, we will inform the ordering provider.
Step 4
Following the appointment, a consultation overview is sent to the referring provider summarizing the discussion, plan, & recommendations.
