Step 1
Referring provider will download and fill out the genetic counseling referral form:
Step 2
Provider sends the referral form, along with any relevant medical documents not already provided with the order to the contact listed at the top of the referral form.
Step 3
The GeneDx patient counseling team will contact the patient to schedule an appointment.
Step 4
Following the appointment, a consultation overview is sent to the referring provider summarizing the discussion, plan, & recommendations.
